Barbara DeCicco is the daughter of George DeCicco, New York mobster and longtime Capo in the Gambino crime family. DeCicco is the older brother of former Gambino Underboss Frank DeCicco, who was killed in a car-bomb meant for his Boss John Gotti, organized by then current Boss of the Genovese crime family, Vincent "Chin" Gigante, and Lucchese crime family leaders Vittorio "Vic" Amuso and Anthony "Gaspipe" Casso as revenge for the murder of former Gambino crime family Boss, Paul Castellano, a strong ally of both the Genovese and Lucchese crime families, according to Wikipedia. Gotti took over as Gambino boss after the slaying and George DeCicco becoming his Underboss.

The greeting card pictures Winnie the Pooh on the front captioned "Today I'm thinking / happy thoughts of you." Printed inside "And tomorrow I'll have / more happy thoughts / that I haven't thought of yet!"

Gotti has handwritten inside the card, in full, "Barbara, I hope this quick card finds you and everyone well and with clear mind set. As for me, I feel great! I hope this finds you all mended and feeling 100%. I also hope Mary is doing well - Both of you are in my thoughts always. Sorry I haven't been writing lately - I'm in between trips and my stuff takes forever to catch up _ I fall behind and then don't know where to start. Barbara, I want to use this card to wish you, Gina, Reynold, your father, Brother & Georgie a very healthy and happy thanksgiving! I'll write a letter in the next couple of days. My Love to all - Stay well and strong! With Much Love, Johnny." Gina and Reynold were Barbara's children.

In 1998, John Gotti was diagnosed with throat cancer and was transferred from the U.S. Penitentiary in Marion, Illinois, to the U.S. Medical Center for Federal Prisoners in Springfield, Missouri, for surgery. From time to time, without notice, he'd be transferred from Marion to Springfield for tests. In this letter he complains that "I'm in between trips and my stuff takes forever to catch up." While the tumor was removed in 1998, the cancer was discovered to have returned two years later and on September 13, 2000, ten months after he wrote this letter, Gotti was transferred from Marion back to Springfield, where he died on June 10, 2002.
